COVID-19 Symptomatic Testing Program
PPS has adopted and is implementing the Oregon Health Authority (OHA) onsite testing of symptomatic student and staff program.  The testing program is designed for staff or students that have symptoms consistent with COVID-19 while at Franklin High School.  These tests will not be used for students or staff that are asymptomatic. Symptomatic students should NOT be brought back onsite for testing.
Testing Consent

• Tests will only be used to test students with parent/guardian consent on file.
• Please complete this COVID-19 Testing Consent Form 

Completed forms can be emailed to Franklin’s MESD Nurse Kay at [email protected] OR can be brought to Franklin by the student and placed in a private “Consent Form” box at any entrance or the main office. We are also mailing paper forms home.
